The motivation of those involved in the website’s development is to help everyone to have choice and control over their lives. The cost of development has been funded from charitable donations. The website is run on a not-for-profit basis, generating only sufficient income to cover its costs. Privacy will be strictly protected. No information will be sold.

Through the “Website”, MyADRT aims to help individuals and professionals understand and take decisions about care towards the end of life, offering guidance to individuals on the preparation of an Advance Decision to refuse treatment and an Advance Statement of preferences and wishes. The motivation of those involved is to help everyone to have choice and control over their lives.

1. YOUR CONSENT

1.1 By providing us with your personal information, including contact details and sensitive personal data such as your address or details about your health, you consent to the collection and use of any information you provide in accordance with this privacy policy.

1.2 We do not intend to collect personal details about children on this Website. If you are under 16, always check with a parent or guardian before giving personal information to anyone online.

2. WHO WILL BE HOLDING YOUR PERSONAL INFORMATION?

2.1 myADRT, as data controller, and the operator of our server (the data processor) will hold the information you provide via the Website.

3. WHAT INFORMATION DO WE COLLECT?

3.1 The personal information that we collect from you is limited to the following information about you:

date of birth;
email address;
address, postcode;
financial details (where you are making payments or donating to us);
phone number;
NHS number;
GP details;
ethnicity/details about your health;
information about the way you use our Website, so that we can improve the effectiveness of our campaign (see our Cookies Policy see below for more information);
the content of your Advance Decision and Advance Statement.

4. HOW DO WE COLLECT INFORMATION?

We will obtain personal information from you in a number of ways, for example when you:

enquire about our activities, the Website or us;
register with the website or donate to us; or
contact us through the Website or any other channel.

5.1 We will use your personal information in a number of ways including to:

help you use what the Website offers, including to produce downloadable documents;
provide you with any information you have requested;
provide or administer activities relating to our services or the services offered on the Website;
provide you with information relating to us or information that we think might be of interest to you, including information about our events and activities;

6. WILL WE EVER SHARE YOUR PERSONAL INFORMATION WITH ANYONE ELSE?

6.1 We will not hand your personal information to third party organisations without your consent, except as set out in clause 6.2 to 6.4.

6.2 We may select a data processor to process your information on our behalf – the software provider that hosts the Website. The data processor is authorised to see and use your information but only to fulfil their contractual obligations to us and are not permitted to use it for any other purpose. We retain full responsibility for how your personal information may be used by such agents.

6.3 We may use information gathered from users of the website for research purposes. Anonymity will be strictly respected.

6.4 Given the purpose of myADRT is to quickly convey your care wishes when time is of the essence, myADRT necessarily makes details you have shared about yourself available for example via scanning of your unique QR code a paramedic would be able to quickly identify you and access your ADRT. You are free at any time to withdraw your ADRT and all details myADRT holds about you from the myADRT database.
6.5 In exceptional circumstances we may share your information when we are legally required to do so.

7. FINANCIAL TRANSACTIONS

If you decide to make a donation to us, or purchase one of our products, your debit or credit card details will be used only for this transaction through our secure online payment system.

8. HOW DO WE PROTECT PERSONAL INFORMATION?

8.1 We place great importance on the security of your personal information and we always try to take appropriate precautions to protect it.

8.2 Despite all of our precautions however, no data transmission over the internet can be guaranteed to be 100% secure. So, whilst we strive to protect your personal information, we cannot guarantee the security of any information which you disclose to us and you must understand that you do so at your own risk.

9. CHANGES TO YOUR DETAILS OR THIS POLICY

9.1 If your personal details change, please help us to keep your information up to date by notifying us at the email address at the beginning of this policy.

9.2 We may revise or update this Privacy Policy from time to time to reflect changes in our activities or for other purposes. If we do so, we will contact you by email.
